Introduction

This Consent to Telehealth Services (“Consent”) explains the nature, benefits, risks, and limitations of receiving healthcare services through electronic communications. By using LifinityRx's Services, you acknowledge that you have read, understand, and voluntarily agree to the terms outlined below.

LifinityRx facilitates access to licensed healthcare providers affiliated with Beluga Health, P.A. and similar professional entities. These providers may deliver consultations, assessments, and treatment recommendations through telehealth technology rather than in-person visits.

What Is Telehealth?

Telehealth is the delivery of healthcare services using electronic communications when the patient and provider are not in the same physical location. Through LifinityRx, telehealth interactions may include:

  • Asynchronous (store-and-forward) communication, such as submitting a health questionnaire that a provider reviews and responds to at a later time;
  • Real-time communication via secure text, audio, or video;
  • Electronic transmission of medical records, health information, and prescriptions.

Potential Benefits

Telehealth offers several potential benefits, including:

  • Access to licensed healthcare providers without the need for an in-person office visit;
  • Convenient, timely evaluations from your home or other private location;
  • Reduced travel time and associated costs;
  • Faster access to treatment when clinically appropriate.

Risks and Limitations

As with any form of healthcare, telehealth has limitations. By consenting, you acknowledge and accept the following:

  • Your provider will not perform a physical examination, which may limit the information available for clinical decision-making;
  • Telehealth consultations may not be appropriate for all medical conditions, and your provider may determine that an in-person visit is necessary;
  • Electronic communications, while secured with reasonable safeguards, carry some risk of interception, technical failure, or delay;
  • In rare cases, a lack of available health information may result in an incomplete assessment, misdiagnosis, or a delay in treatment;
  • Technology disruptions, including poor connectivity or system outages, could interrupt or prevent a consultation.

Your Responsibilities

To ensure safe and effective care, you agree to:

  • Provide accurate, complete, and current information about your health history, medications, allergies, and symptoms;
  • Inform your provider of all other healthcare providers involved in your care;
  • Follow up with your provider or seek in-person care if your condition worsens or does not improve;
  • Contact emergency services (call 911) immediately in the event of a medical emergency — telehealth is not a substitute for emergency care;
  • Use the Services in a private setting where you can openly communicate with your provider.

Prescriptions

If a licensed provider determines that medication is appropriate based on your consultation, a prescription may be issued and sent to a licensed U.S. pharmacy for fulfillment. You understand that:

  • Not all consultations will result in a prescription;
  • Prescribing decisions are made at the sole discretion of the treating provider;
  • Some medications may be compounded by certified pharmacies to meet individual needs and are not evaluated or approved by the FDA;
  • You are responsible for using any prescribed medication as directed and for reporting any adverse reactions to your provider.

Privacy and Confidentiality

Your health information is protected in accordance with applicable federal and state laws, including HIPAA. For full details on how your information is collected, used, and safeguarded, please refer to our Privacy Policy and the Notice of Privacy Practices issued by the affiliated professional entity providing your care.

Right to Withdraw Consent

You may withdraw your consent to telehealth services at any time by contacting us at care@lifinityrx.com. Withdrawing consent will not affect the legality of any services provided prior to withdrawal, but may limit your ability to continue receiving treatment through LifinityRx.

Agreement

By using LifinityRx's Services, you confirm that you have read this Consent to Telehealth Services, understand the risks and limitations of telehealth, and voluntarily consent to receive healthcare services through electronic communications as described above.
